Once we receive your information, we will send you a secure link to our client portal. Through the portal, you will be able to create an account and complete our intake forms electronically.
After your intake forms are submitted, our Clinical Director will review your information. A member of our team will then contact you to discuss next steps and scheduling.

We want to be transparent about how we handle insurance. In the past, our team has called insurance companies to verify eligibility and benefits on behalf of clients. However, we have learned that the information provided is often incomplete or inaccurate. Because of this, we will no longer verify benefits for clients.
If you plan to use insurance benefits, we encourage you to contact your insurance company directly. When contacting your insurance company, please provide them with our Tax ID (45-1619072) and Group NPI (1760727978). Please know that using our company name alone may result in inaccurate information.
